Excavation Planners help busy excavators prepare for calls to our operators.  They'll help you make sure you have all the information we need, and they're especially useful in large operations that assign calls to someone other than the excavator.  Download the form and print as many copies as you need. Emergency Excavation Guides list the information we need to process emergency locate requests 24 hours a day, 365 days per year.  WARNING: By law, emergency locate requests must involve a "crisis situation which poses an imminent threat or danger to life, health, or property and requires immediate action." Declaring an emergency under any other circumstances may subject you to legal penalties.

Damage reports may be filed directly with the State Police via fax using this standard State Police Underground Utilities Complaint Form. (requires Adobe Acrobat).

For Use by LA One Call Members Only

Violation reports are used (by members only) to provide information for review by law enforcement agencies for possible violations of the Louisiana Underground Utility/Facility Damage Prevention Law.  Louisiana One Call screens and submits these reports.

Excavator notifications are used (by members only) to directly inform excavators who appear to be working without a Louisiana One Call ticket that their activities may be hazardous and in violation of state law.  Part of the form is given to the excavator and the other part is mailed or faxed to Louisiana One Call.  When we receive a properly completed form our damage prevention manager will mail a warning letter and offer a free safety presentation.  Printed copies of the excavator notification form that include a convenient postage-paid business reply card are available to members on request from our damage prevention manager.
